Hmmm... don't know why you're having this problem, unless you have 
not selected disc-at-once in the preferences and 0 seconds between 
tracks. I have done what you want (continuous mix) with Toast 4.0.2 
and a Glyph (Yamaha guts) 4/2/6 (yeah, it's a bit old) and it's 
worked out fine... even mastered a disc for pressing this way. The 
subindex thing is indeed weird, but as few (if any) players use 
subindexes anymore there's not much you could do with it anyway.
